After Wrennie posted the first link I "googled" and got lots of hits.  Some were PDF files while others were done in Word.  Some had logos and others were just plain and easily modified.  Most of the Word formatted ones encouraged you to download and modify where the PDF ones limited you to printing and filling in with pen and pencil.

Guess we need to print a few and see what we like and dislike and customize it to what we want.  Should be simple enough with Word and/or Excel.

Forgot to mevtion this sight also has alot of ghost stories as well along with other exciting stuff.

It has been a while since I have been there. Looks like they have been doing some modifying to the site. Thanks for the link, Jayrod. :) The form that we have now is similar to what they have. :grinnnn:
